Evidence-based medicine (EBM), by its ability to decrease irrational variations in health care, was expected to improve healthcare quality and outcomes. The utility of EBM principles evolved from individual clinical decision-making to wider foundational clinical practice guideline applications, cost containment measures, and clinical quality performance measures. At this evolutionary juncture one can ask the following questions. Given the time-limited exigencies of daily clinical practice, is it tenable for clinicians to follow guidelines? Whose or what interests are served by applying performance (...) assessments? Does such application improve medical care quality? What happens when the best interests of vested parties conflict? Mindful of the constellation of socially and clinically relevant variables influencing health outcomes, is it fair to apply evidence-based performance assessment tools to judge the merits of clinical decision-making? Finally, is it fair and just to incentivize clinicians in ways that might sway clinical judgment? To address these questions, we consider various clinical applications of performance assessment strategies, examining what performance measures purport to measure, how they are measured and whether such applications demonstrably improve quality. With attention to the merits and frailties associated with such applications, we devise and defend criteria that distinguish between justice-sustaining and justice-threatening performance-based clinical protocols. The goal of the 2010 Ontology Summit was to address the current shortage of persons with ontology expertise by developing a strategy for the education of ontologists. To achieve this goal we studied how ontologists are currently trained, the requirements identified by organizations that hire ontologists, and developments that might impact the training of ontologists in the future. We developed recommendations for the body of knowledge that should be taught and the skills that should be developed by future ontologists; these (...) recommendations are intended as guidelines for institutions and organizations that may consider establishing a program for training ontologists. At first the confrontation was purely intellectual, as writers on ethical theory (within phi losophy, theology, or other humanistic disciplines) turned their attention to topics from the (...) world of medical practice. Recently the confrontation has grown more intense. The ap pointment of clinical ethicists in hospitals and other health care settings is an accelerating trend in North America. Concomitantly, those institutions involved in training peo ple in clinical ethics have added organized exposure to the world of practice, in the form of placement requirements, to the normal academic course load. In common with other dis ciplines, bioethics has begun to see clinical training as a con dition of didactic theory and apprenticeship. (shrink)

Theories such as effective altruism contend that people are morally obligated to give to charitable organizations that will efficiently do the most net good. The assumption is that aiding people who are most in need will create the most good; yet, it may be more inefficient to reach those most in need. In response, I outline my Inefficiency Principle in which efficiency has less moral weight when aiding those lacking in basic capabilities, and efficiency has more moral weight when aiding (...) those who are lacking in more complex capabilities. This principle acknowledges the obstacles in assisting those most in need while sustaining the moral importance of efficiency. (shrink)

How developing a more expansive, non-formal conception of reason produces richer ethical understandings of human situations, explored and illustrated with many real examples. In Re-Reasoning Ethics, Barry Hoffmaster and Cliff Hooker enhance and empower ethics by adopting a non-formal paradigm of rational deliberation as intelligent problem-solving and a complementary non-formal paradigm of ethical deliberation as problem-solving design to promote human flourishing. The non-formal conception of reason produces broader and richer ethical understandings of human situations, not the simple, constrained depictions (...) provided by moral theories and their logical applications in medical ethics and bioethics. Instead, it delivers and vindicates the moral judgment that complex, contextual, and dynamic situations require. Hoffmaster and Hooker demonstrate how this more expansive rationality operates with examples, first in science and then in ethics. Non-formal reason brings rationality not just to the empirical world of science but also to the empirical realities of human lives. Among the many real cases they present is that of how women at risk of having children with genetic conditions decide whether to try to become pregnant. These women do not apply the formal principle of maximizing expected utility (as advised by genetic counselors) and instead imagine scenarios of what their lives could be like with an affected child and assess whether they could accept the worst of these scenarios. Hoffmaster and Hooker explain how moral compromise and a liberated, extended, and enriched reflective equilibrium expand and augment rational ethical deliberation and how that deliberation can rationally design ethical practices, institutions, and policies. The Healer's Art curriculum is one of the best-known educational strategies to support medical student professional identity formation. HART has been widely used as an elective curriculum. We evaluated students’ experience with HART when the curriculum was required. All one hundred eleven members of the class of 2019 University of New Mexico School of Medicine students were required to enroll in HART. We surveyed the students before and after the course to assess its self-reported impact on key elements of professional (...) identity formation such as empathy towards patients and peers, commitment to service, and burnout. A majority of students reported positive effects of the course on their empathy towards other students. This finding was significantly associated with self-reported willingness to have elected the course had it not been required. One-half of respondents reported positive effects on their empathy towards future patients. At least one-quarter to one-third of respondents reported positive influences on commitment to service, conceptions about being a physician, and self-perceived burnout. Students report benefits on their professional identity formation after participating in a required course on humanism. Empathy-building among peers is one valuable outcome of such curricula. According to the coercive approach to distributive justice, the coercive nature of the political state requires justification in the form of distributive benefits owed only to members of the state. In this paper I analyze and dismiss traditional objections to the coercive approach, and I proceed to raise two novel objections. First, according to my equivocation objection, I contend that the coercive approach’s leap from coercive burdens to certain distributive benefits is based on an equivocation. When this equivocation is clarified, (...) the conclusion of the coercive approach is undermined. Attempts to circumvent the equivocation inevitably lead to my second objection, the coercion as cooperation objection. Through this objection it becomes clear that the coercive approach collapses into the cooperative approach, an approach to distributive justice that may change the scope of distributional obligations beyond that of the political state. Smoking consequences are seen disproportionately among low-SES smokers. We examine the self-regulatory strategy of mental contrasting with implementation intentions as a smoking reduction tool and whether its effectiveness depends on subjective-SES. This pre-registered online experiment comprised a pre-screening, baseline survey, and follow-up. Participants reported past-week smoking, subjective-SES, perceived stress, and were randomized to an active control or MCII condition. Data were collected via MTurk, during the U.S.’ initial wave of COVID-19. Participants were moderate-to-heavy smokers open to reducing or quitting. The (...) primary outcome was self-reported smoking reduction, computed as the difference between recent smoking at baseline and follow-up. The secondary outcome was cessation, operationalized as self-reported 7-day point-prevalence abstinence at follow-up. Among those low—but not high—in subjective-SES, MCII improved smoking reduction by an average of 1.09 fewer cigarettes smoked per day, though this effect was not conclusive. Similarly, quitting was descriptively more likely for those in the MCII than control condition, but the effect was non-significant. Per an exploratory analysis, we observed that stress significantly moderated the condition effect, such that MCII facilitated reduction among those experiencing high, but not low stress. Consistent with prior findings that MCII works best in vulnerable populations, MCII may be more effective for smoking reduction among high-stress than low-stress individuals. These findings contribute to growing research on income-related health disparities and smoking behavior change tools. (shrink)

Vulnerability does not mean much for our contemporary morality. It is antithetical to our emphasis on individualism and rationality; it requires that we attend to the body and to our feelings. Yet only by recognizing the depth and breadth of our vulnerability can we affirm our humanity.

Ethical business practices have been widely prescribed, but why? Consumers views on unethical business practices have been studied, but possibly more important to marketers and researchers are consumer actions and reactions to unethical business practices and the businesses themselves. Do consumers react negatively, or in such a way as to "punish" the unethical business? If so, what is the nature and extent of the punishment? This research seeks answers to these questions by examining consumer reactions, such as complaining and switching, (...) to instances of unethical business practices. Using equity theory, this research proposes that consumers should be willing to tolerate some unethical behavior as long as they feel their investments and outcomes remain proportionately equal. Consumers who perceive that their outcome/investment ratio is proportionately unequal to their comparison other will respond by switching or complaining. In this research consumers were exposed to two types of service failures with different levels of service recovery in vignettes. Costs incurred by the consumer during the service transaction were also manipulated in the vignettes. Significant differences were found for complaint behavior in the failure recovery vignettes tested. Specifically, voice complaint was higher in the high cost service encounters in both types of services tested. The recovery attempt used (ethical, unethical, or none) led to significance differences in the variables of complaint, voice complaint, satisfaction, and quality. Higher satisfaction and quality ratings were found for the ethical recovery attempt and higher intentions to use complaint and voice complaint in the unethical recovery attempts. One significant interaction between cost and recovery attempt was found. Intentions to use voice complaint were higher in the high cost situations of ethical and unethical recovery, while approaching equality at the no recovery attempt. (shrink)

Philosophical notions of humanitarianism – duties based in beneficence that apply to humanity generally – are largely focused on personal duty as opposed to official development assistance, or foreign aid, between nations. To rectify this gap in the literature, I argue that, from the point of view of donor nations, their humanitarian obligations are met when they have given enough of their fair share of resources, and from the point of view of recipient nations, they have received enough when they (...) have reached a threshold of capabilities. I conclude that a future theory of humanitarian obligations ought to take into account the disparate, and often conflicting, interests of nations as benefactors or as recipients of aid. (shrink)

This article examines the responses of Parisian noble women to campaigns for women's rights in France of the early Third Republic. The methodology of the article is based on the works of Pierre Bourdieu. His concept of the habitus is used to analyse the effects of class and gender in noble women's attitudes to French feminisms before the First World War. The conditioning of Parisian noble women explains their resistance, indeed often outspoken opposition, to feminists' demands. These female aristocrats supported (...) their own oppression within a social order governed by the state, the scientific and medical establishments, the expectations of family, and the Catholic Church of the time. One of the largely untapped potentials of Sausserian semiotics is the ability it provides to examine shifts in the cultural meanings attached to objects and ideasacross time. To explore this potentiality, we trace the intertextual evolution of one of the most enduring mythic figures, the vampire. Our analysis begins with ancient texts and moves forward in time to contemporary cinematic and televised depictions of the vampire. We document the deployment of the vampire as a vehicle carrying oppositional meanings as it (...) moves through various eras. For example, vampires may connote both racism and racial resistance, homophobia and gay acceptance, women’s subjugation and women’s empowerment. By using the vampire to mediate contested, liminal regions of cultural discourse, storytellers have constructed increasingly complex and multivocal portraits of this figure. At present, vampire narratives have come to resemble an alternative universe of human morality and behavior, complete with family structures, emotional depth, and interpersonal conflicts. (shrink)

In May 2016, we launched Research Integrity and Peer Review, an international, open access journal with fully open peer review (reviewers are identified on their reports and named reports are published alongside the article) to provide a home for research on research and publication ethics, research reporting, and research on peer review. As the journal enters its third year, we reflect on recent events and highlights for the journal and explore how the journal is faring in terms of gender and (...) diversity in peer review. We also share the particular interests of our Editors-in-Chief regarding models of peer review, reporting quality, common research integrity issues that arise during the publishing process, and how people interact with the published literature. We continue to encourage further research into peer review, research and publication ethics and research reporting, as we believe that all new initiatives should be evidence-based. We also remain open to constructive discussions of the developments in the field that offer new solutions. (shrink)

Oxford: Oxford University Press, 2019. vii + 220 pp. Cloth, $70.00—Berryman’s goals in Aristotle on the Sources of the Ethical Life are threefold: to establish that Aristotle practiced what contemporary philosophers call metaethics; to refute the idea that Aristotle justified those ethics by recourse (...) to human nature, understood as an “external point” for “establishing substantial ethical truths from a value-neutral perspective”; and to demonstrate that, for Aristotle, the source and justification for ethical demands is human practical reason.Locating the source of normativity in practical reason does not mean that there are no “standards of truth other than human beings as a species.” Positioning Aristotle against both “Archimedean” naturalisms as well as relativisms and subjectivisms, Berryman sees Aristotle’s appeals to nature as delimiting “the kinds of normative positions that could guide practical reason,” and practical reason as responsive to biological constraints. While the postmodern critique of universals provides important insights, it also leaves us in an unacceptable position---lacking solid justification for moral judgments and political action, and unable to generalize about human experience. I argue that the best response to relativism lies in a new humanism. Any new humanism must be "post-humanist"---taking into account valid critiques of past humanisms, incorporating multicultural voices, and building upon an understanding of the common human condition that does not erase or ignore difference. My project is (...) to present a re-conceptualized notion of the "human condition" that meets these conditions, to serve as a basis for a new humanism. ;As a starting point, I take up the framework found in Jean-Paul Sartre's later Marxist-Existentialist works. These are helpful in two respects. First, as he attempts to reconcile Marxism and Existentialism, Sartre re-examines central categories of human experience and ultimately arrives at an understanding of the human that transcends both essentialism and anti-essentialism. Second, I argue that the methods Sartre develops anticipate crucial postmodern insights without losing moral and political ground, and that a reworking of these methods enables us to theorize beyond postmodernism today. My project also draws upon Feminism, Critical Race Theory, Postcolonialism, African Philosophy, and the Frankfurt School. ;The central elements of my articulation of the human condition include: a rethinking of the relationship of the individual and the social; an account of the social constitution of the individual; a new approach to understanding freedom and necessity in human experience; and a reconsideration of social identities such as race, class, and gender. In recent years, calls for a new humanism have arisen from a variety of voices across the spectrum of philosophy, expressing frustration with outdated models of the human that cannot account for the richness of our social being. The postmodern deconstruction of the human now requires a reconstructive moment. In response, the author articulates a new and explicitly posthumanist humanism using the framework developed by Jean-Paul Sartre in his later Marxist-Existentialist works. Sartre's unique dialectical and hermeneutical methods allow us to (...) reconceptualize the human beyond traditional dichotomies of individual/social and freedom/necessity. The author argues that the individual and the social should be understood as existing within a dynamic, co-constituting interrelation, and that individual autonomy is not at odds with, but rather fundamentally enabled by, the social. This essay follows Richard Schaeffler in identifying Kant’s moral philosophy as a possible framework for a Catholic theology of hope. Whereas Ernst Bloch criticized Kant for failing to sever his theory of hope from its religious ties, Jürgen Moltmann criticizes Kant for failing to appreciate the true meaning of Christian hope for the kingdom of God. The present essay argues that Moltmann neglects, as much as Bloch did, the significance of God to Kant’s account of the kingdom. A Catholic theology (...) of hope would have to lie somewhere in-between the atheist utopianism of Bloch and the evangelical certainty of Moltmann, and that is precisely what Kant’s concept of hope does. This article traces the gendered consequences of changes in the problem of combining work and family caused by the collapse of state socialism in former East Germany. The transition to capitalism made the trade-offs between work and family more extreme, amplified the experiential distance between work and family, and increased the perceived social value of work relative to family activities. These processes highlighted gender stratification: Women's labor power was devalued just as the value of paid employment increased, and women were (...) increasingly excluded from work just as women's family roles seemed to lose value for society. This study suggests that feminists should conceptualize postsocialist transitions not only as changing labor market conditions and state policies but also as shaping gender relations through changing the meanings and experiences of work and family activities. We expect schools to be a safe haven, but after more than a decade of targeted school violence prevention laws and safety plans, students are still marginalized and bullied to the point of despondence, retaliation, and even suicide. This thoughtful exploration of what makes a school a safe place is based on the understanding that violence is a continuum of acts and attitudes–subtle to overt–that have a negative effect on how students feel and learn.
